What is a thigh lift?

Treatment Overview

A thigh lift, or thighplasty, removes excess skin and fat from the thigh and tightens what remains. It is performed under general anaesthetic and takes around two to three hours, with most patients home the same day or after one night.

The great majority of thigh lifts are inner (medial) thigh lifts, because that is where skin gathers after weight loss and where the rubbing happens. Where laxity extends around the outside of the leg and onto the hip and buttock, an outer or circumferential lift is needed instead — a considerably bigger operation.

Be wary of anyone who offers you a thigh lift without discussing liposuction first. If your thighs are heavy but your skin still retracts, liposuction alone gives a better result with three tiny incisions and no scar down your leg. Thigh lift scars are the least forgiving in body contouring — they sit in a high-friction, high-tension area — so the operation is worth doing only when skin, not fat, is genuinely the problem.

Thigh Liposuction, Mini, Inner or Outer Thigh Lift

Your surgeon will ask you to stand, then gather the skin on the inside of your thigh and let it go. If it springs back, the problem is volume and liposuction will treat it. If it stays gathered, the skin has lost its elasticity and has to be removed. They will then check how far the laxity travels — upper third only, the whole inner thigh, or round onto the hip — because that decides the length and direction of the scar.

1. Thigh Liposuction — from £4,500
For heavy thighs with skin that still retracts. Fat is removed through incisions of a few millimetres and the skin shrinks down over the smaller leg. No long scar, compression for six weeks, back at a desk within a week. Where the skin is borderline, liposuction first and a lift later is often the sensible order — it is far easier to remove skin from a slimmer thigh than to undo a scar.

2. Mini Thigh Lift — from £8,500
For laxity limited to the upper inner thigh. A short incision sits in the groin crease, hidden by underwear, and the skin is lifted towards it. The scar is the shortest and best concealed of any thigh lift — but the technique only lifts the top of the leg, and used on a thigh that needs more it simply pulls the groin tight and leaves the knee end untouched.

3. Inner (Medial) Thigh Lift — from £8,500
The standard operation after major weight loss. Where laxity runs the length of the inner thigh, the groin incision is extended down the inside of the leg towards the knee — a vertical thighplasty. It is the only technique that removes skin along the whole thigh, and it is the reason the scar is long: the skin has to come out lengthways, not just at the top. Liposuction is usually used alongside it.

4. Outer or Circumferential Thigh Lift — quoted at consultation
Where loose skin continues around the outside of the thigh onto the hip and buttock, the lift is extended around the leg, often as part of a lower body lift with an abdominoplasty. A much larger operation with an overnight stay, and the right one only for substantial circumferential laxity.

Am I suitable for a thigh lift?

Ideal Candidate for Thighplasty

Thigh lift is the body contouring operation with the least forgiving healing, so candidate selection matters more here than almost anywhere else. Your surgeon will look at:

  • A stable weight for at least six months, ideally twelve. Further weight loss creates new laxity; weight gain stretches the result and the scar.
  • A BMI generally under 30. This threshold is applied more strictly for thigh lifts than for arms, because tension across the groin incision rises sharply with thigh volume.
  • Skin that does not retract on the pinch test — otherwise liposuction is the better operation.
  • Not smoking or vaping for at least six weeks before and after. Wound breakdown in the groin is the commonest complication of this procedure and nicotine multiplies the risk.
  • No significant lymphoedema or previous groin lymph node surgery without specialist review.
  • Realistic expectations about the scar. It is long, it sits in a difficult area, and it is the one scar in body contouring most likely to widen. Accepting that in advance is what separates satisfied patients from disappointed ones.
  • A plan for six weeks of restricted activity — no wide leg movements, limited walking early on, and compression worn continuously.

Stable Weight
Results are best and most durable at a stable weight you can maintain. Your consultant will advise on timing if you are still losing weight.

Realistic Expectations

The Pinch Test Standing, your surgeon gathers the inner thigh skin and assesses whether it retracts. That single test decides between liposuction and a lift.

How Far It Travels They then map how far the laxity extends — upper third, full inner thigh, or round onto the hip — which sets the length and direction of the incision.

Where the Scar Will Go You will be shown the planned scar on your own leg and told plainly that the groin is a difficult area to heal and that this scar is the one most likely to widen.

Preop Checks Routine bloods and medical review. After major weight loss we check protein, iron, vitamin B12 and vitamin D and correct anything low — groin wounds are unforgiving on poor nutrition.

Preparation Stop smoking and vaping at least six weeks beforehand. You will be measured for compression garments and asked to arrange help at home, since walking is restricted early on.

Admission Admitted by our nursing team and seen by your surgeon and anaesthetist. Incisions are marked with you standing, because thigh skin falls completely differently lying down.

Follow‑Up Wounds are checked within the first week and again at two weeks, then at six weeks, three months and a year. Groin wounds are reviewed more often than other body contouring sites, by design.

First Two Weeks Rest with your legs supported, short gentle walks, no wide leg movements, no lifting. Expect swelling and tightness. Desk‑based work at ten to fourteen days.

Two to Six Weeks Walking builds gradually. Driving usually at three weeks. Manual work from four to six weeks. Compression continues throughout.

Six Weeks Onwards Exercise resumes at six to eight weeks, cycling and swimming later than walking. Swelling settles over three to six months and the scar fades and flattens over twelve to eighteen months with silicone and sun protection.

Do I need a thigh lift or just liposuction?

It depends on whether your skin retracts, not on how heavy your legs feel. Your surgeon gathers the inner thigh skin and lets go: if it springs back, liposuction alone will work, from £4,500, with no long scar. If it stays gathered, removing fat alone will make it hang more and the skin has to come out. Where it is borderline, doing liposuction first and reassessing in six months is often the better order.

Honestly, this is the least forgiving scar in body contouring. It runs in the groin crease and, for a vertical thighplasty, down the inner thigh towards the knee. The groin is warm, damp, constantly moving and under tension, so this scar is the one most likely to widen, stretch or take longer to settle than you would like. It is hidden by underwear and by most swimwear. Modern closure anchors the deep tissue to the fascia to take tension off the skin, which improves it considerably — but nobody can promise you a fine line here.

Two to three hours for an inner thigh lift under general anaesthetic. Liposuction alone is under an hour. Most inner thigh lifts are day cases; circumferential lifts usually involve one or two nights.

Ten to fourteen days for desk‑based work, longer than an arm lift because sitting is uncomfortable early on. Four to six weeks for work involving standing, walking or lifting. Driving at around three weeks.

More restrictive than most people expect. For the first two weeks you walk with small steps, avoid opening your legs wide, and get in and out of chairs and cars in a particular way you will be taught. Compression is worn day and night for six weeks. It is not a painful recovery so much as an awkward one, and patients who have planned for that cope with it well.

Bleeding, infection, fluid collection (seroma), and above all delayed wound healing or partial wound separation in the groin, which is more common here than at any other body contouring site. Also scar widening or downward migration of the scar, numbness on the inner thigh, asymmetry, and swelling of the lower leg if lymphatic channels in the groin are disturbed. Smoking, a high BMI and poor nutrition raise all of them. Your surgeon will discuss the risks specific to you.

Often, and it is usually cheaper: the second procedure in the same anaesthetic is charged at half price. Thigh lift with abdominoplasty is common after major weight loss. Your surgeon will weigh total operating time and your health before combining — longer operations carry more risk, particularly of blood clots, and thigh lifts already restrict early mobility.

Not reliably. Tightening the skin can make dimpling look less obvious on the treated area, but cellulite is caused by fibrous bands tethering the skin to deeper tissue and a thigh lift does not release them. Anyone promising cellulite removal from a thigh lift is overselling it.

Radiofrequency and ultrasound devices give a modest improvement on mildly lax skin. They cannot remove excess skin. If you can gather a handful of loose inner thigh skin, no device will take it away.

Rarely. Funding is restricted to documented, recurrent skin breakdown that has not responded to treatment, and criteria vary by integrated care board. Most patients in Greater Manchester find funding is declined and choose to go privately.

The skin removed does not come back. The thigh will still age, and significant weight change will alter the result, but with a stable weight most patients keep the shape they leave with. Scars continue improving for a year to eighteen months.
